Course Description:

Health outcomes for the population incorporate collaboration of health professional and community partners.  The purpose of Population Health is to impact the health outcomes of a group.  Utilizing the APTA Vision, a strategy will be presented and an implementation plan established to optimize movement to improve health outcomes within the population.

ObjectivesAt the completion of this course you will be able to:

  • Identify community partners for collaboration on healthy population activities.
  • Describe and design policies and programs to guide health outcomes through community-clinic linkage.
  • Apply best practices for health promotion and wellness among the community partners and residents.
  • Design community based health and wellness plans to engage the community in optimizing movement
