How Do I Report...?
This page is for reporting concerns related to harassment, discrimination, and conduct matters. It will guide you through a series of questions to help you file the correct report and ensure it reaches the appropriate office.

This site will assist you in filing your report to ensure it is routed to the appropriate person who can assist you.
Does the nature of your complaint involve any type of sex-based discrimination, harassment, or violence, such as sexual assault, dating violence, stalking, or any behavior that interferes with you receiving your education?
Does the nature of your complaint involve the person being treated unfairly because of protected characteristics, including race, religion, sex, national origin, disability, sexual orientation, or gender identity or expression?
Does the nature of your complaint involve a possible violation of the Code of Conduct against an employee (faculty and staff) or contractor?
Does the nature of your complaint involve a possible violation of the Code of Conduct against a student?
