Coursedog Academic Forms
Stockton is transitioning several academic forms from fillable PDFs and physical or
emailed signatures to digital workflows in Coursedog.
Faculty and staff will use Coursedog to initiate, review, and approve these requests. Each form will be routed automatically to the appropriate reviewers, creating a more accessible, efficient, and transparent process.

Why forms are moving to Coursedog?
Improved Accessibility
Moving from PDF forms to online workflows supports Stockton’s efforts to meet Title II ADA web-accessibility requirements.
Automatic Routing
Coursedog sends each request to the appropriate preceptor, program chair, Dean, Academic Advising, or Registrar reviewer.
Easier Tracking
The person who initiates the form can see where it is in the approval process without contacting multiple offices.
Centralized Approvals
The request, supporting information, comments, and approval history remain together in one system.
What is changing for faculty and staff?
Coursedog approvals replace physical and emailed signatures.
When a request reaches your workflow step, you must sign into Coursedog to review it and record your decision. Replying “approved” to an email or providing a signature outside Coursedog will not move the request to the next step.
Already use Coursedog?
You may already use Coursedog for Scheduling or Curriculum. Academic forms are located in a different area of Coursedog and may require additional permissions. If you can sign in but cannot access the appropriate forms or approval queue, submit a Coursedog Access Request.
Can students submit these forms?
No. The appropriate instructor or preceptor initiates the form on the student’s behalf.
Students will not initiate, approve, or sign these forms in Coursedog.
Students should discuss their request with the appropriate instructor or preceptor. If the request is appropriate, the instructor or preceptor will initiate the form on the student’s behalf. After submission, the form will move through the required faculty and administrative approval steps. Students are not responsible for obtaining signatures or moving the form between offices. Submission does not guarantee approval; all required workflow steps must be completed.
Can I approve a request by replying to the notification email?
No. You must sign into Coursedog and record your decision in the workflow.
I already have Coursedog access. Why can’t I see the forms?
Existing Scheduling or Curriculum access may not include academic forms. Submit a Coursedog Access Request so your permissions can be reviewed.
What happens if I do not act on a request?
Approvers have 5 days to act on a request. After the 5 day deadline, the request returns to the author.
