Re-Matriculation Form
The Re-matriculation form is for former undergraduate students only and is used to request matriculation to be reinstated to complete an undergraduate degree or certification, or to pursue a second undergraduate degree without formally reapplying to Stockton.
Complete this form to have your matriculation reinstated if you plan to:
1) Complete an unfinished undergraduate degree; or
2) Pursue a second undergraduate degree.
A $50 non-refundable re-matriculation fee will be charged to your account (even if no registration occurs).
IMPORTANT:
- If you attended another college since leaving Stockton, please apply through the Office of Admissions.
- If you are interested in returning for a nursing degree, contact the Nursing Program directly for information about admission. At Stockton the nursing program is a limited, competitive, and fall semester entry only program with special admission requirements.
- If you are not pursing a degree and plan to take only a few courses, please complete the Non-Matriculated Student Application.

Do not use this form if you have been dismissed. If you are dismissed, you can find information about appealing for reinstatement to the University under Dismissal & Readmission Information. You may contact the Office of the Provost at appeals@stockton.edu.
- If you completed a minimum of 120 credits toward a degree at Stockton; were in good academic standing when you left Stockton, i.e., had at least a 2.0 grade point average (GPA), and was not subject to disciplinary action when you left the University you may be eligible appeal forAcademic Amnesty.

If your current name is different from when you last attended Stockton please submit a Legal Name Change Request.
- Forms must be processed prior to the start of the term. It is recommended that the form be submitted two weeks prior to the start of the term for which you are applying.
