Forms relating to the Bursar's Office are below. Forms for other departments (i.e., Financial Aid, Student Records, etc.) can be found on that department's web page.

Financial Aid
To access financial aid information including the Free Application for Federal Student Aid (FAFSA), loans, work study or other financial aid forms.

Housing

Housing Application
For new housing applicants ONLY.  Students currently living on campus do not submit this application. Complete and submit the online application for the appropriate term from the Student Life tab in goStockton in the Residential Life box (center bottom).  Housing deposit is required.

Housing Cancellation
To cancel housing due to a documented medical necessity, financial hardship or other unforeseen circumstance. Complete and submit the Housing Contract Cancellation form from the Student Life tab in goStockton in the Residential Life box (center bottom). Termination fee applies.

Housing Renewal
To renewal housing for students currently living on campus.  Complete and submit the form from the Student Life tab in goStockton in the Residential Life box (center bottom).

Meal Plan

Meal Plan Form 

All students (Commuters or Residential) may use the link above to submit a new meal plan or a meal plan change.

Replacement Check

To request a replacement student refund in the event of lost, destroyed or never received check.

Student ID Card (Osprey Card)

Add Funds to Osprey Card
To transfer funds from your account onto your Student ID (Osprey Card).  Sufficient funds must be available before for submitting the request.  From the Bursar's/Payments tab click on Add Funds to Osprey Card.

Lost or Stolen Card
Students must report lost/stolen ID cards through GetFood and then go to Student Records (CC-203) for a new ID.  There is a $25.00 fee for replacement IDs.

Perkins Loan

Exit Interview
Online counseling session required before you withdraw, graduate or drop below half-time to help you manage student loans after
college.

Tax Forms
1098-T tax forms are issued mid-January (prior to January 31) each year.  Tax forms can be obtained through your goStockton account
(on the Bursars/Payment tab or the Alumni tab) or from Web Services if you are a former student.

Tuition Waiver

Employee
For Stockton employees ONLY.  Please refer to Stockton's Policies and Procedures for more information regarding the procedure
tuition assistance and waiver for employees.

Spouse, Partner or Dependent of an Employee 
To be completed and submitted by a Stockton employee for a spouse, legal partner or dependent.  Please refer to Stockton's
Policies and Procedures for more information regarding the procedure for tuition waivers for a spouse, partner or dependent of
employees.
